I am an out of network provider with all commercial insurance companies and am not a Medicare provider. For patients that means that fees for appointments are due at the time of service. Payments may also be submitted electronically via Zelle or through the patient portal.

It depends on your insurance plan details. Many private insurance companies offer partial reimbursement for services provided by out of network physicians. You will receive a a detailed statement with the billing and diagnostic codes necessary to submit for partial reimbursement if you choose. Please call the toll free number on the back of your insurance card or visit your insurance company portal to obtain details of the reimbursement rate (if applicable) and any deductible of your specific plan.
I am not a participating provider with Medicare and do not accept assignment. I have opted out of Medicare and in order to see Medicare patients certain paperwork must be completed indicating understanding and agreement that Medicare will not reimburse for treatment with a provider who has opted out. If you have Medicare now or you enroll in Medicare after beginning treatment, please let me know before scheduling an appointment so we can discuss further.
